The Admission Application Process Checklist is a great tool to help you understand the steps involved in applying for Admission to East Orchard Christian Academy. Some of the items on the checklist are links to the related document. Just hover your cursor over each word to see if it is a link.

Complete and sign the application.

Both parent(s)/guardians are required to sign in two separate places on the application.

PArental contact information

Include a copy of the student's birth certificate. The birth certificate must accomopany the application at the time of submission.

All students applying for PreK-5th Grade must complete and include the Student Medical Form with the immunizations attached when the application is submitted.

All new students will need a completed physical from your doctor before attending the first day of school. This physical can be no older than one year old from the start of school year.

Include a copy of the previous year final report card and current year report card.​

Complete and sign the Transcript Request Form and return it with the application. Records will be requested by EOCA if the student is accepted, after the completion of the current school year.​

Each student must have on file a completed financial agreement signed by the parents or guardian.

This must be signed by the parent or guardian, dated, and sent to your student's current teacher. The teacher must mail the form back to EOCA.

This agreement outlines our attendance policy, discipline policy,and parent committment to our school as well as our faith statement.

This form is for families to list the areas they would like to help with at the school and skills they are willing to share with our students.
